Practically, how do we achieve this? The general method most commonly applied is to reference to the loudest sounds the system has to cope with and adjust the gain so that these pass through the system at levels comfortably below clipping. This method essentially ignores the noise floor, assuming that most equipment has a low enough noise floor for this not to be a major consideration. If the noise floor is a problem and there is no more headroom in the system - and this usually only occurs in material with a wide dynamic range such as classical music or some theatre work - then the signal will need to be compressed.
